چکیده

The study aimed at Aim evaluation the cerebral hemodynamics in cirrhotic patients by transcranial Doppler ultrasonography and its relation to hepatic encephalopathy. This study is a prospective study , 50 subjects attended El Hussein university hospital, inpatient and outpatient clinic are classified into three groups, group I(10 subjects as a healthy control),group II (20 subjects with liver cirrhosis without hepatic encephalopathy) and group III(20 subjects with liver cirrhosis with hepatic encephalopathy). All patients included in the study underwent a full history taking, complete physical examination, liver functions tests, blood ammonia level,C.B.C,blood urea and serum creatinine and pelvi-abdominal ultarasonography. Transcranial Doppler done for all patients participated in the study using a 2 MHz transducer. The results showed that as regard Trans cranial Doppler ultrasound parameters, we found a statistical significant difference between the three groups as regard Pulsatility Index (P ˂ 0.001), Breath-Holding Index (P ˂ 0.001) ) , Maximum Flow Velocity(P ˂ 0.001) and Mean (P ˂ 0.001), whereas Pulsatility Index, , Maximum Flow Velocity and Mean are higher in patient with Hepatic Encephalopathy, but Breath-holding index is low, there is no statistical significant difference (P > 0.05) between them as regard Minimum Flow Velocity. Conclusion: Transcranial doppler ultrasonography is easy, rapidly done not invasive method for evaluation of cerebral hemodynamic in cirrhotic patient and predection of hepatic encephalopathy.

Cerebral hemodynamic in patients with major β-Thalassemia using transcranial Doppler sonography

Background: β-thalassemia, a severe form of anemia, is an inherited blood disorder characterized by growth retardation, splenomegaly, and bone abnormalities. Complications related to treatment-induced iron overload also affect the quality of life of patients with major β-thalassemia. Some recent studies indicated cerebral hemodynamic disorders and increased risk of stroke in these patients. The...

Use of Transcranial Doppler for Monitoring Hepatic Encephalopathy

Fulminant hepatic failure (FHF) is an acute and severe disease that can lead to death by the development of encephalopathy. Due to the severe alterations of coagulation that go along with this clinical picture, it may be contraindicated to place a sensor for monitoring intracranial pressure (ICP); it is for this reason that the option of the transcranial doppler (TCD) arises as an auxiliary met...

Cerebral Hemodynamics and Cognitive Function in Cirrhotic Patients with Hepatic Encephalopathy

Aims. To investigate cerebral hemodynamics in cirrhotic patients with HE and to observe effects of treatment in cerebral hemodynamics and correlations among ammonia, cerebral hemodynamics, and cognitive function. Methods. There were four groups: healthy controls (group 1), cirrhosis without HE (group 2), cirrhosis with MHE (group 3), and cirrhosis with OHE (group 4). Ammonia and cerebral hemody...

Evaluation of the role of TLR4 in endotoxin-induced hepatic encephalopathy in rats with biliary cirrhosis

Background: Hepatic encephalopathy is defined as a neuropsychiatric brain dysfunction in acute or chronic liver failure. Infection and inflammation have crucial role in its pathophysiology. The purpose of our study was to demonstrate the relationship between toll-like receptor 4 (TLR4) expression and the encephalopathy induced with endotoxin in biliary cirrhotic rats. Methods: The present stud...
